Please be aware that Delta Dental PPO fees only apply when a PPO participating dentist is providing care. While most of our providers are in the PPO plus Premier networks, some are Delta Dental Premier only. There may be additional out-of-pocket costs for you if a non-ppo dentist provides care for you or your family. If you have a PPO Only plan or prefer a PPO provider care for you or your family, it is your responsibility to request a PPO provider at the time of scheduling all appointments.

We are happy to accept out-of-network insurance and will submit claims, on your behalf, to most other insurance carriers as a courtesy. You will be expected to pay for the estimated portion your insurance does not cover at the time of service. If your insurance carrier does not reimburse Haas Dental Associates for services rendered, rather they pay the subscriber directly, you will be expected to pay for the services in full at the time of service.
